Website Privacy Policy

One Village Family Chiropractic Community

Effective date: October 21, 2026

Our commitment to your privacy

Your trust matters to us. This policy explains how One Village Family Chiropractic Community (“One Village,” “we,” “us”) collects, uses, discloses and protects information through our website and related chiropractic services. It applies to our Crestwood and Highlands chiropractic locations. Independent practitioners and external websites may have their own privacy policies.

We handle patient health information under Alberta’s Health Information Act (HIA) and applicable regulations. Alberta’s Personal Information Protection Act (PIPA) applies to other personal information where applicable. Our chiropractic privacy and record practices are also governed by the College of Chiropractors of Alberta (CCOA) Standards of Practice and Code of Ethics.

Information we collect

When you contact us, request an appointment, subscribe to updates or use an online booking service, we may collect your name, email address, telephone number, appointment preferences and the information you choose to provide. During chiropractic care, we collect information reasonably necessary for assessment, care, record keeping and administration, including health history, examination findings, care notes and payment records.

Our website and its hosting services may process technical information such as your IP address, browser and device information, pages visited, dates and times, and security logs. See the cookies and website services section below for details.

Why we use information

We use information for reasonable, identified purposes: responding to inquiries; arranging appointments and reminders; providing and documenting chiropractic care; issuing receipts and administering payments; maintaining website security and functionality; responding to access or correction requests; and meeting legal and professional obligations. We limit collection, use and disclosure to what is necessary and legally authorized.

We do not sell personal or health information. We do not use identifiable patient health information for advertising or promotional content without the appropriate separate authorization.

Consent and your choices

We explain the purposes for collection and obtain consent where the law requires it. Some collection, use or disclosure of health information is authorized by the HIA without consent, including certain care and legally permitted administrative purposes. This website policy is not consent to chiropractic care or a blanket authorization to disclose your health records.

You may contact us to withdraw consent for optional uses or express wishes about disclosure of your health information. We will explain any legal limits and any effects on the services we can provide. Withdrawal does not undo an earlier lawful use or disclosure and does not require destruction of records we must retain.

Online inquiries and booking

Please provide only the information needed to arrange an appointment or ask a general question. Do not send detailed health histories, identification documents or sensitive clinical information through ordinary website forms, email or social media. Contact the clinic to arrange an appropriate method for sharing clinical information.

The Highlands booking link directs you to Bookedin, which has its own privacy policy. Information entered into third-party booking services is handled by those services as well as by us where we receive it. Contact us if you prefer to arrange an appointment by telephone. Website messages are not monitored continuously and must not be used for emergencies.

Email, text messages and newsletters

We use your contact preferences for appropriate administrative communication. Ordinary email and text messages can carry privacy risks, including misdelivery and access by others using your device. Please tell us if your contact details or preferences change. When communicating identifying health record information electronically, we use safeguards appropriate to the information and applicable professional requirements.

Optional newsletters and promotional messages require a lawful basis under Canada’s anti-spam legislation. You can unsubscribe using the message’s unsubscribe option or by contacting us. Declining promotional messages does not affect your access to chiropractic care.

Cookies and website services

Cookies are small files used by websites to support functions and remember preferences. Website services, embedded content and external links may also process technical information. Our site includes external booking links, Google Maps content and links to social media and video services. Those providers’ privacy practices apply when you interact with their services.

[Before publication, insert the verified cookie and tracking details here: services/providers in use; information collected; purposes; cookie duration; whether analytics or advertising technologies are enabled; and how visitors can accept, refuse or change optional tracking choices.]

You can manage cookies through your browser settings. Blocking cookies may affect some website functions. We do not treat simply visiting this page as consent to all optional tracking.

When information may be shared

Information may be accessed by authorized clinic personnel and service providers only as necessary for their duties and authorized purposes. Providers may support website hosting, booking, communications, records management or payment processing. We remain responsible for our legal obligations when using these services and require appropriate privacy and security arrangements.

Health information may be shared with other healthcare providers or persons when authorized by law or by valid consent. Information may also be disclosed when required or permitted by law, including lawful regulatory inquiries, court orders and applicable reporting duties. We do not give family members, employers or insurers unrestricted access to your health records.

Any use of identifiable patient photographs, stories or acknowledgements for public communication requires appropriate separate written permission and must also meet CCOA requirements.

Service providers and processing locations

Some service providers may store or process information outside Alberta or Canada, where it may be subject to the laws of that jurisdiction. We assess these arrangements and apply applicable legal requirements and safeguards.

[Before publication, identify verified processing countries and purposes for each relevant provider, including hosting, forms, booking, email/newsletters, analytics and clinical systems. Insert the contact or link for obtaining further information about foreign service providers. Do not state that all information stays in Canada unless verified.]

Protecting and retaining information

We use reasonable administrative, physical and technical safeguards appropriate to the sensitivity of the information, including restrictions on access, staff confidentiality expectations and secure handling of records. No website or communication system can guarantee absolute security.

Patient health records are retained for at least 10 years after the last entry. If the patient was under 18 at the last entry, records are retained until at least age 28. Longer retention may be necessary for legal or professional reasons. Other personal information is retained only as reasonably needed for its purpose and applicable legal requirements, then securely destroyed or appropriately anonymized.

Access, corrections and representatives

You may request access to your personal or health information and ask us to correct information you believe is inaccurate or incomplete. Send a written request to the privacy contact below. We may verify your identity and any representative’s authority before releasing information. Access is subject to the exceptions in applicable law.

We respond to HIA access requests within 30 days and PIPA access requests within 45 days, unless a lawful extension applies. We explain any permitted fees, refusal or extension and relevant review rights. Patient record access is not withheld because an account is unpaid.

For children or individuals represented by another person, we verify lawful authority and consider the individual’s capacity, privacy rights and applicable legal requirements. Being a relative does not automatically authorize access.

Privacy incidents and concerns

If a privacy incident occurs, we take steps to contain it, assess the risk and meet applicable reporting and notification requirements. Where an HIA breach creates a risk of harm, we notify the affected individuals, Alberta’s Information and Privacy Commissioner and the responsible Minister as soon as practicable in accordance with the law. For information governed by PIPA, we report breaches meeting its real risk of significant harm threshold to the Commissioner and meet applicable notification requirements.

You may raise a concern with our privacy contact. You may also contact the Office of the Information and Privacy Commissioner of Alberta at https://oipc.ab.ca to make a privacy complaint or request a review. Concerns about professional conduct may be directed to the CCOA at https://theccoa.ca.

Before publishing — clinic and website review

This page is for the clinic and website developer. Do not publish it as part of the public policy.

  • Complete the effective date and bracketed cookie/tracking and processing-location sections. Confirm Dr. Wendy Coburn as the designated privacy contact and verify all contact details.
  • Confirm the policy matches actual practices: website forms, hosting, backups, Bookedin, newsletter platform, payment providers, clinical systems and any independent practitioners. Check whether embeds load tracking before a visitor chooses to interact.
  • Verify consent controls, newsletter unsubscribe functions, limited form fields, secure transmission and safeguards for electronic health records. Patient consent does not transfer the chiropractor’s security obligations.
  • Review service-provider contracts and any required HIA information manager agreements, privacy impact assessments and information-sharing arrangements. A posted policy alone does not establish compliance.
  • Confirm that the stated no-sale and promotional-use commitments are followed. Review public patient stories and acknowledgements for both written permission and CCOA advertising requirements.
  • Use this Alberta policy rather than a HIPAA-compliance claim. HIPAA applies to defined U.S. covered entities and business associates; obtain a separate applicability assessment if the practice has such an arrangement. POPA primarily governs Alberta public bodies and is not the general privacy law for a private chiropractic website.
